ANXIETY & OCD THERAPY SACRAMENTO
Your brain can learn a new response.
I’m Dmitri Primavera, a Sacramento therapist specializing in evidence-based care for anxiety and OCD—using ERP, ACT, and mindfulness to help you feel more grounded. In Fair Oaks and online across California.
Evidence-based careWhen anxiety and OCD start to take over your life…
It's hard to explain what anxiety really feels like—the constant scanning, the need to make sure nothing's been missed, your body clenched tight without realizing it. You double-check, replay, and analyze. Any relief is brief, and then the questions start again.
It’s not that you’re oblivious to what's happening. You probably understand anxiety better than most people ever will. But this knowledge doesn't quiet your mind. It keeps dragging you back into the same loops, no matter how logical or rational you try to be.
These patterns are treatable. With the right structure and practice, your brain can build a different response, with less noise and more ease.
We Can Work On
Therapy Approach
My approach is relational and collaborative: we'll work together in a therapeutic relationship defined by safety, empathy, and respect.
-
The gold standard for OCD. We systematically face distressing thoughts, situations, or stimuli—learning to resist compulsions at a manageable pace.
-
We build psychological flexibility—making room for difficult thoughts and feelings while moving toward what matters most to you.
-
Practical attention skills that help you notice experiences without getting swept away by them.
-
Tools to reduce shame and self-criticism, so you can relate to yourself with the same kindness you’d offer a friend.
-
We identify unhelpful thinking patterns and underlying beliefs, and practice more balanced, workable ways of understanding situations, ourselves, and the world.
-
We build awareness of triggers and urges, teach competing responses, and use stimulus control to reduce hair pulling and skin picking.
-
A tailored plan addressing sensory, cognitive, emotional, motor, and environmental factors to reduce episodes and increase choice.
-
A research-backed approach for trauma and PTSD. We gradually revisit difficult memories in a safe environment to reduce their intensity over time.
-
Effective techniques land best in a safe relationship. I pay attention to patterns of connection and repair, tailoring the work to your history and nervous system.
-
A brief, evidence-based trauma treatment using structured writing exercises to process and reduce trauma-related distress.

Dmitri Primavera, LCSW
Anxiety has a way of convincing you that vigilant effort is the answer—that if you think harder, prepare more, or avoid the wrong thing, you’ll find relief. Most of the people who come to work with me have tried this strategy for years. They’re thoughtful, capable, and exhausted by how much energy it consumes.
I understand this struggle on a personal level, and I know what actually helps it change. My background includes expert training and intensive clinical work. The methods I use are active and evidence-based, grounded in current research and honed from years of focused experience treating anxiety and OCD.
What matters most to me is that therapy feels clear and effective. We take a structured approach, guided by collaboration and steady practice, so you can recognize what anxiety is doing and respond differently. My goal is to bring expertise and calm direction to the process, so you can find greater ease, clarity, and confidence.

I’m a private-pay provider and don’t bill insurance directly, I can give you a monthly superbill to submit for possible out-of-network reimbursement.
I also keep a few reduced-fee openings for clients who need them. Contact me to see what’s available.
-
Yes. I see clients in person at my Fair Oaks office (Sacramento area) and offer secure telehealth sessions for clients anywhere in California.
-
Not at all. Many clients come to me without a formal diagnosis. You don’t need to know if what you’re experiencing is anxiety, OCD, or something else—you just need to know it’s getting in the way of your life.
Part of our work will be clarifying what’s happening and how to approach it.
-
A consult call is a free 15-minute phone call where we talk briefly about what you're struggling with, what you're hoping to get out of therapy, and whether it makes sense to move forward.
It's not a therapy session, and there's no pressure to commit. It’s a chance for both of us to see if it feels like a good match.

I’m a Licensed Clinical Social Worker (LCSW) in California with advanced training in evidence-based treatment for anxiety and OCD.
I earned my MSW from California State University, Sacramento, and completed five years of clinical training at The Anxiety Treatment Center of Sacramento, an intensive outpatient program specializing in severe OCD-spectrum conditions. I’ve continued my education through the International OCD Foundation (IOCDF), including consultation with expert Jon Hershfeld, LMFT.
Before becoming a therapist, I earned an international MBA from the University of South Carolina and worked in global companies—experience that now informs my practical, collaborative approach to care. I also hold a B.A. in Psychology and Political Science from UC Davis.
